As a Service Coordinator with our human services nonprofit is dynamic and fulfilling. Every day you work collaboratively with participants to determine the enrollment of services and activities based on their employment and educational goals, as well as their skill levels and potential barriers. Using your excellent attention to detail, you assess the participant’s needs, considering their perspective, background, and any immediate barriers that may hinder their progress. Your expertise and observations play a vital role in identifying appropriate services and addressing barriers effectively. You guide participants in making informed decisions that align with their goals, providing ongoing support and advocacy throughout their journey.
Additionally, you participate in the development, implementation, and monitoring of individualized Service Plans, scheduling regular meetings with participants to review progress and make necessary adjustments. Post-program, you follow up with participants to monitor their progress and address any potential barriers that may arise. Administrative tasks, such as entering case notes into databases, are also part of your responsibilities. This multifaceted role with our human services nonprofit offers opportunities to make a lasting impact on individuals’ lives, supporting their growth and achieving their aspirations.

QUALIFICATIONS
- Bachelor’s degree in human services, social work, education, or comparable work-related experience
- 2+ years in case management or service delivery
- Experience and/or a demonstrated desire to work with economically disadvantaged youth (18-24), homeless, justice-involved, and/or public assistance recipient populations
- Excellent communication and data entry skills
